Description:
The Veteran Outreach Specialist plays a critical role in helping veterans access the care they deserve by guiding them through the Community Care authorization process and supporting monthly veteran admissions. This role involves proactive outreach to veterans, Veteran Affairs offices, and community partners, ensuring that veterans are aware of available resources and services. The ideal candidate will have strong communication skills, attention to detail, and a passion for supporting veterans.
Assist Veterans: Guide veterans through the Community Care authorization process, ensuring they can access the necessary services.
Document Interactions: Update and maintain accurate records in Salesforce to ensure up-to-date documentation.
Outreach: Conduct proactive outreach efforts to veterans, Veteran Affairs offices, and community partners to increase awareness and engagement.
Effective Communication: Communicate effectively with veterans, families, VA representatives, and internal teams to streamline the admission process.
Stay Informed: Keep up-to-date on VA policies, Community Care requirements, and resources available to veterans.
Customer Service: Provide empathetic and professional customer service to veterans in need.
Military Experience (preferred) or strong familiarity with veteran services and VA processes.
Experience in case management, outreach, or healthcare coordination (preferred).
Proficiency in Salesforce or similar case management/documentation software.
Excellent communication and interpersonal skills.
Ability to work independently while meeting monthly outreach and admission goals.
Strong organizational skills with the ability to multitask and prioritize responsibilities.
Commitment to maintaining confidentiality and professionalism when handling sensitive veteran information.
