The Program Coordinator will provide day to day oversight and coordinate the different elements to SAMHSA AIM 2.0.The project will provide patient-centered Substance Use Treatment and supportive services to Black and Latinx men and women aged 18 and over in Upper Manhattan and the South Bronx with a Serious Mental Illness (SMI) or Co-occurring Disorder (COD).Project AIM’s goal is to reduce the incidence of HIV, HCV, and improve overall mental health outcomes for individuals with SMI or COD among the population of focus and to reduce disparities in accessing and remaining engaged in treatment by conducting targeted outreach to connect clients to substance use, mental health, and recovery support services as needed, including the Seeking Safety and Wellness Self Management Plus evidence-basedinterventions.The program will be located at the 125th street location andwilltarget surrounding areasas well as neighborhoods in Upper Manhattan and the Bronx.The Program Coordinator will provide supervision, orientation, and training to Care Manager and Prevention Navigator.They will also developand overseeoutreach strategiesin conjunction with the teamto engage and enroll clients for initial assessment and reassessment, linkage to treatment and supportive services,and retention in programming.
Responsibilities
- Lead start up activities related to the program, including hiring, training staff, developing tracking mechanisms and outreach/linkage/retention strategiesin conjunction with Managing Director
- Oversee ongoing datacollectionutilizing theGPRA/NOMStoolfor Baseline and Follow Up Assessment
- Collaborate with ProgramEvaluator and Data Manager to solicit feedback and collectevaluation data fromparticipantsofevidence-basedinterventions
- Seeking Safety and WSM+ and other EBIsin order to assess effectiveness of the intervention.
- Oversee participant recruitment and implementation of evidence-based group interventions, including supervision of group facilitators
- Prepare bi-annual and annual reports for funders in collaboration with the Data Manager, ProgramEvaluator and Managing Director
- Ensure program deliverablesand goals are met by tracking progress on a monthly basis and utilizing that data to troubleshoot challenges and inform strategies
- Participate in weekly supervisory sessions with the Managing Directorand provide weekly supervision to program staff
- Assists with development and implementation of program events, including collaboration with leaders in the targeted community
- Coordinate participation in community events, such as health fairs, LGBT events,etc
- Provide presentations for internal and external programs to promote the program servicesand develop referral partnerships
- Maintain essential datain SPARS system and internal care manager databasefor reporting and program evaluation
- Lead quality assurance efforts in the program to ensure client services are delivered effectively and data is documented accurately and in a timely manner
- Ensure compliance withagency and departmentpolicies and procedures
- Assist as needed with Intake and Follow up assessments, case management activities including linkage to care and integrated case conferences,coordinate with other service providers regarding linkage and referrals
- Participate in agency wide efforts to reach strategic goals related to Ending the Epidemic initiative, including participation in committeesas directed
- Participate inreoccurringinteragency meetings, as well as external community meetings to improve integrated care andmaintain relationships with community partners
- Participate in department activities such as new staff onboarding, YPS Pride Committee, CQI efforts,etc
- Participate in Housing Works advocacy events (minimum 4 per year July-June)
- Perform other duties as assigned
