Physical Therapist

We are seeking a compassionate and skilled Physical Therapist (PT) to deliver personalized in-home care for patients in Sunnyvale, CA. 𝐑𝐞𝐬𝐩𝐨𝐧𝐬𝐢𝐛𝐢𝐥𝐢𝐭𝐢𝐞𝐬:
Provide physical therapy services according to the physician’s plan of care.
Assist patients with exercises, gait training, and prosthetic use.
Develop individualized treatment plans to relieve pain, restore function, and enhance mobility.
Maintain timely clinical records and documentation.
Participate in team meetings and educational programs to further develop skills and knowledge.
Qualifications:

𝐋𝐢𝐜𝐞𝐧𝐬𝐮𝐫𝐞: Licensed Physical Therapist in California.
𝐄𝐝𝐮𝐜𝐚𝐭𝐢𝐨𝐧: Doctorate or Master’s degree in Physical Therapy.
𝐄𝐱𝐩𝐞𝐫𝐢𝐞𝐧𝐜𝐞: Minimum of one year of PT experience required.
𝐂𝐞𝐫𝐭𝐢𝐟𝐢𝐜𝐚𝐭𝐢𝐨𝐧𝐬: Current CPR certification.
𝐎𝐭𝐡𝐞𝐫 𝐑𝐞𝐪𝐮𝐢𝐫𝐞𝐦𝐞𝐧𝐭𝐬: Valid driver’s license

𝐊𝐞𝐲 𝐀𝐭𝐭𝐫𝐢𝐛𝐮𝐭𝐞𝐬:
Strong problem-solving abilities and adaptability to individual patient needs.
Ability to work independently while also collaborating with a multidisciplinary care team.
Clear and effective communication with patients, families, and healthcare professional.
