Licensed Practical Nurse Clinical Coordinator

As a Licensed Practical Nurse Clinical Coordinator, you will play a key role in providing high-quality care to patients in their homes and helping to ensure seamless coordination between patients, healthcare teams, and physicians. Your role will involve administrative tasks, patient interaction, and supporting the development of a compassionate, dedicated healthcare team.

Key Responsibilities:

• Mission & Philosophy Promotion: Promote Elara Caring’s mission and policies to ensure quality of care.

• Coordination and Administration: Review and clear HCHB coordination notes, process administrative workflows, and obtain physician orders (verbal, written, or faxed) for review and approval.

• Patient Support: Schedule follow-up appointments, provide health summaries to case managers, assist with pre-authorizations, and address patient concerns.

• Chart Audits: Perform audits as required by Elara Caring policy and verify receipt of Medicare provider non-coverage notices.

• Privacy & Confidentiality: Ensure patient and staff privacy and confidentiality in accordance with HIPAA.

• Field Visits: Perform direct patient care (field visits) as needed.

• Additional Duties: Undertake other duties and projects as assigned by supervisors.

Required Qualifications:

• Education: Graduate of an accredited nursing school.

• License: Current, unrestricted LPN license valid for the state of work.

• Experience: Minimum 1 year of experience as a nurse in a clinical setting.

• Mobility & Travel: Ability to travel within branch/office coverage area and possess a dependable vehicle, valid driver’s license, and auto insurance.

• Physical Requirements: Ability to sit, stand, bend, lift, and move intermittently, with the capacity to lift 50-100 lbs.

• Bilingual: Spanish/English proficiency is a plus.
