Hospital Social Worker

Requirements:
- Education: Master’s Degree in Social Work (MSW)
- Licensure: NYS LMSW license required
- Experience: 1+ year as an LMSW, ideally in inpatient medicine, surgery, or acute rehab. Other preferred settings include medical or rehab facilities, inpatient psych, or ER.

Key Responsibilities:
-Assess Patient Needs: Evaluate patients’ social and emotional needs to inform care planning.
-Counseling & Support: Provide therapeutic guidance for patients and families.
-Discharge Planning: Develop safe discharge plans and coordinate community resources.
-Crisis Intervention: Respond to immediate patient crises, offering support and referral to appropriate services.
-Patient Advocacy: Facilitate communication between patients, families, and the healthcare team.
-Documentation: Keep thorough and timely patient records.
-Collaborate with Medical Team: Engage in interdisciplinary meetings to align care plans.
-Resource Guidance: Connect patients to relevant community and financial resources.
-Weekend Coverage: Available on weekends as needed for continuous patient support.
