The Hospice Admissions Manager position is open in our Ontario location and surrounding areas. This is a Full-time position.
Requirements of the Hospice Admissions Manager:

- A minimum of 2 years Hospice clinical experience.
- Current Registered Nursing license in the State of California.
- Strong leadership abilities.
- Excellent communication skills.
Hospice Admissions Manager Duties Include:

- Demonstrate support and fulfillment of the agency’s mission, vision, values.
- Implements and monitors agency initiatives to achieve strategic goals.
- Communicates, collaborates and coordinates in a positive and professional manner with all peers, leaders and staff. Cooperates with all departments by sharing information as requested or needed in a timely manner.
- Supports a healthy work environment by promoting effective communication, collaboration, cooperation, and conflict resolution with all departments and all levels of staff.
- Maintains ongoing public relations and communication with physicians, hospital and facility staff, and referral sources regarding patient care and all available agency services.
- Works cooperatively with the acute hospitals, skilled nursing facilities, and medical groups in the community to plan and coordinate Hospice services for referred patients.
- Provides ongoing supervision to admission department staff to maintain quality care. Stays current with staff evaluations and supervisory visits.
- Provides for, motivates and encourages staff development and growth.
- Conducts counseling, teaching and guiding of Hospice personnel.
- Oversees planning and implementing orientation of new personnel and students.
- Coordinates and conducts regular department meetings.
- Participates with other departments to ensure that patients are informed of their hospice benefits and admitted to the hospice program when appropriate.
- Serves as a consultant to staff and outside agencies. Recommends and/or assists with problem solving.
- Provides direct supervision to the after-hours clinical staff.
- Reviews clinical documentation on an ongoing basis to ensure compliance with eligibility requirements and maintenance of quality care and documentation standards.
- Oversees the para-professionals involved in the referral process:
- Scheduling appropriately and timely
- Arranging transportation, delivery or DME and medications
- Appropriate communication with the patient, family and referral source as needed
- Responds to all referrals with a sense of urgency
- Monitors the timeline of referral to admission
- Oversee the professionals involved in the admission process:
- Evaluation and admission is done timely utilizing appropriate triage skills
- Referrals are responded to with a sense of urgency to provide care
- Patients and families are informed of hospice services and the benefit of these services to them
- Monitors the conversion rate of referrals to admission status of individual RN’s and Social Workers
- Referrals are discussed with the admission staff while they are with the patient if RN/SW feels patient does not meet eligibility requirements or the patient is receiving treatments that are not usually covered by the hospice benefit.
- Evaluates, recommends and assists with the development of hospice policy and procedures.
- Is motivated and takes initiative with regards to the development of the Hospice Admission department and remains current in understanding of Medicare/Medi-Cal Hospice Regulations.
- Participates in Performance Improvement Program.
- Participates in agency goals and objectives.
- Maintains professional accountability; manages time effectively.
- Communicates effectively with public, co-workers, subordinates and superiors. Understands proper channels of communication within the agency.
- Maintains patient/family confidentiality as per HIPAA regulations.
