Citizens Memorial Healthcare Foundation is recruiting a Director of Social Services at Colonial Springs Healthcare Center

Description of department: Colonial Springs Healthcare Center is a 134-bed long-term care, skilled nursing center that includes a 34-bed memory care unit, as well as a 16-bed high-functioning memory care unit designed to meet the specific needs of individuals with Alzheimer's disease as well as other forms of dementia.

A typical day will include: Provides Social Services to residents; actively and consistently contributes to facility operations communications; behaves in a manner consistent with the mission and objective of CMH and performs other duties as requested.
