Description:


The Associate Director for Post-Acute Care is a key leader in our organization who is responsible for engaging with customers to understand their desired outcomes and provide recommendations on workflow and adoption of our solutions. The ideal candidate has extensive clinical and operational management experience in multiple care settings including acute and post-acute care such as home health care, hospice, skilled nursing facilities and inpatient rehabilitation. The Associate Director for Post-Acute Care also has expertise in designing and executing post-acute network strategy and alignment with alternative payment models, such as bundling, ACOs, and DCEs. To be effective, our customers and prospective clients may need to adopt new technology and workflows. As the Associate Director for Post-Acute Care, you will bring to bear a thorough understanding of all PointClickCare resources to support value creation and a user’s ability to hold themselves accountable to the rigors of an alternative payment model. Additionally, the Associate Director for Post-Acute Care can assess and optimize transitions of care, care coordination, readmission reduction, and discharge planning across the continuum of care. Key Responsibilities
 

  • Evaluate customer data, workflows, and goals to offer tailored recommendations to build scalable processes.
  • Educate others within the organization by offering resource support and onboarding on best practice post-acute care models, in order to help our customers improve outcomes and drive value.
  • Through direct relationships, conferences, and analysis of market activity, develop and maintain deep understanding of our customers, their pain points, use cases, and clinical workflows; support efforts to scale this knowledge across internal teams and PCC’s customers through ad hoc and regular updates.
  • Working with Senior Care, Product and Product Marketing, provide strategic insights for our product team to help guide innovation in our solutions.
  • Provide thought leadership as a keynote speaker at our Care Collaboration Conferences.
  • Stay abreast of relevant regulations and policies related to post-acute care management and value-based care to help ensure organizational compliance.
  • Helping to champion the intersection of senior care and the acute and health plan space. Guiding customers to see the value created from cross divisional adoption of solutions.
     

Qualifications & Skills
 

  • Deep domain expertise and at least 3 years direct experience with the clinical, financial, and quality infrastructure and needs of the post-acute industry (include risk-bearing entities, like ACOs and health plans, or senior living, skilled nursing facilities). Preference: 3-5 years' experience in either an acute, post-acute setting, or with a health plan.
  • Experience in negotiating provider reimbursement. Preference: Experience in drafting actual VBC arrangements.
  • Bachelor's degree in a healthcare related field. Preference :Master's degree in a healthcare related field.
  • Strong analytical, data, financial, leadership and communication skills.
  • Strong critical and creative thinking and problem solving skills.
